Referrals to Cloud Hub

IMPORTANT INFORMATION:

Cloud Hub is an 'Alternative Provision' setting, and not a school. We are only able to accept 4 full-time pupils (of whom can't have an EHCP or be a 'Looked After Child/LAC'). 

We can accept pupils (who do have an EHCP or be a LAC), on a part-time basis - but for no more than 16 hours each week - and Cloud Hub cannot be their main place of learning. 

Registration Arrangements
Where pupils remain on role at their mainstream school, they will be 'dual registered'.
Permanently excluded pupils will be single registered with Cloud Hub.

 

Step 1: COMPLETE THE REFERRAL FORM

If you are a parent/carer or school/education provider looking for Alternative Provision, please email the completed form as attached above, back to us, providing as much information as possible.

On obtaining the referral form, we will contact you within two working days to discuss the referral and any further information you may wish to give us.

Step 2: A 'SERVICE LEVEL AGREEMENT' IS PREPARED

If the place is accepted, Cloud Hub will create a 'Service Level Agreement' (a form of a contract), to be signed by both settings. Copies of this are to be retained by Cloud Hub and the school/education provider.

 

Step 3: LET'S MEET / COME IN AND VISIT US (from September 2024)

After a place is agreed, we will meet the young person and have a brief and informal chat. We like to meet all our new learners individually, in a convenient location - on-site or off-site (referral charge of £50/hour applies). 

In this meeting, discussions around targets/goals, learning areas to work on (Maths/English/Science if appropriate), potential AQA Unit Award Scheme subjects, activities etc. This helps us to start to build up a picture and learning journey...completely centred around the pupil and their individual needs.

 

Step 4: INDUCTION

Pupils and staff spend time getting to know each other and the environment. This process increases understanding of pupils' backgrounds and needs, supports decision-making and facilitates the development of positive relationships.

Documents are implemented, such as (but not limited to): 

- Risk Assessments

- Pupil Passport

- Personalised Learning Plan

- 'Boxall Profile' / 'Strengths and Difficulties' assessments

- 'About Me' / 'What Helps/Doesn't Help Me' / 'My Strategies' pupil documents

- AQA Unit Award Scheme; preliminary discussion

 

Communication
Communication is key at Cloud Hub. We keep all parents, carers, school staff and external agencies (if necessary), up to date at regular intervals with achievement, attainment and general information. This could take the form of posts via the 'Dojo' app, termly reports, progress reports, celebration certificates etc.

Cloud Hub believe in positive and effective collaboration between parents/carers, schools, Local Authority teams (and of course ourselves), to create high quality provision. 

We welcome visits previous to referral from all parties, and would be happy to accommodate and answer questions you may have.

 

LOCAL AUTHORITIES / IF YOUR CHILD HAS AN EHCP

Generally, the Local Authority in which a child resides will consult Cloud Hub for a place. For further information search 'SEN Local Offer' or 'Alternative Provision' on your Local Authority webpage.
